About the Role
The Carceral Transitions Case Manager (CT CM) is responsible for pre‑release planning and short‑term case management of members releasing from city and county jails, Department of Corrections, Juvenile Rehabilitation, and Juvenile Detention facilities, ensuring they transition safely back into community settings. The role provides short‑term case management services for members with highly complex behavioral health, medical, and social conditions. Working within a multi‑functional team, the CT CM connects with members, correctional facility staff, providers, caregivers, contracted vendors, community resources, and health plan partners to assess the member's reentry into the community, identify care needs, and ensure access to appropriate services to achieve positive health outcomes.
To be successful in this role, you:
Have a background in behavioral health, experience working with diverse populations, or collaborating with justice‑involved individuals.
Have experience with community resources such as Regional Centers, Medicaid mental health and substance abuse services.
Hold a bachelor’s degree or an equivalent combination of education and highly relevant experience.
Have a current, unrestricted license in Washington as a Registered Nurse, Social Worker, or Mental Health Professional.
May hold a Case Manager Certificate.
Essential functions and Roles and Responsibilities:
Identify, locate, and determine custody status of justice‑involved members; advocate and facilitate coordination of resources to help members reach optimum functional levels upon reentry.
Collaborate with city and county jails, Department of Corrections, and Juvenile Rehabilitation facilities and a multi‑functional team to connect with providers, caregivers, vendors, community resources, and health plan partners to assess health status, identify care needs, and ensure access to services upon reentry.
Perform assessment, planning, facilitation, and advocacy through collaboration with the member and facilities.
Meet with justice‑involved members via phone or video conferencing.
Maintain CM records and documents within the JIVA system.
Participate telephonically in discharge planning meetings and interdisciplinary care conferences when appropriate.
Schedule medical, mental health, dental, and/or vision appointments in accordance with release dates and arrange transportation if required.
Communicate with providers regarding discharge plans and status updates.
Coordinate with UM intake coordinators for service referral authorizations for release to home or alternate care settings.
Perform other duties as assigned.

Ability to obtain an NPI number.

Compensation and Benefits:
The position is FLSA Non‑Exempt, eligible for overtime, and offers a 10% annual incentive target. Base pay varies by experience.

Qualifications
Education
Preferred: Bachelor’s or better.
Licenses & Certifications
Preferred: LMHC, LICSW, RN.
