Overview Behavioral Health Case Manager (BHCM) role at COPE Community Services, Inc.
Responsibilities Serve as a point of contact and advocate for members.
Provide face-to-face, phone, virtual, and in-home services as needed by each member’s recovery journey.
Develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner.
Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting progress on the service plan as defined by policies and procedures.
Serve as a point of contact and advocate to ensure ongoing collaboration with relevant parties (e.g., primary care providers, adult probation, other involved service providers), including the communication of appropriate clinical information regarding progress.
Contribute to ongoing collaboration, communication, documentation, and coordination of covered services; track progress of treatment needs and goals.
Minimum Qualifications Bachelor’s or master’s degree, no experience necessary; or
Associate degree plus two (2) years of behavioral health work experience.

Additional Requirements Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.
Unacceptable driving records include (examples): more than two moving violations in the last three years; more than one at-fault accident in the last three years; any major violation (DUI, careless/reckless driving, drag racing, hit and run) within the last three years. Other driving-related violations may be considered unacceptable.
At time of job offer, submission of a 36-month driving record from the DMV is required. Driving records are reviewed regularly for current employees.
