Summary: This position is located in the Department of Behavioral Health at Evans Army Community Hospital in Fort Carson, Colorado.
Duties Serves as expert behavioral health case manager and member of multidisciplinary team of medical and professionals.
Develops, implements, communicates, and coordinates individualized empirically informed patient treatment plans.
Coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting.
Develops and recommends strategies to improve healthcare access.
Monitors, tracks and intervenes as patients transition between levels of care.
Responsibilities and Details Overview dates: Open & closing dates 11/04/2025 to 11/14/2025. Salary: $90,961 to $118,254 per year. Pay scale & grade: GS 12. Location: Fort Carson, CO. Appointment type: Permanent. Work schedule: Full-time. Service: Competitive; Promotion potential: None. 0601 General Health Science.

A Personnel Security Investigation is required.
Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
A Drug Test may be required for this position.
Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
Must possess and maintain a valid stateside driver’s license.
May be required to work other than normal duty hours to support non-duty hour classes.
Must be able to obtain and maintain Basic Life Support (BLS) Training and Certifications from the American Heart Association and the American Red Cross within 6 months of hire.
Licensed as Clinical Social Worker is required.

In order to qualify, you must meet the Behavioral Health Case Manager requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Basic Requirement for Behavioral Health Case Manager
Degree: Bachelor’s or Graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of Behavioral Health Case Manager.
Specialized Experience: One year of specialized experience which includes performing behavioral health assessments; providing individual and group support; and assisting patients with access to healthcare systems and resources. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).
Some federal jobs allow you to substitute your education for the required experience in order to qualify. For the GS-12, you must meet the qualification requirement using experience alone–no substitution of education for experience is permitted.
Additional information Male applicants born after December 31, 1959 must complete a Pre-Employment Certification Statement for Selective Service Registration.
You will be required to provide proof of U.S. Citizenship.
One year trial/probationary period may be required.
