Overview Behavioral Health Care Manager - Part-Time – Fort Health. This role requires an independently licensed clinician; NJ in-person PT position. Salary: $35,000 to $40,000 annually based on experience. Schedule: 20 hours a week; 4 days a week. In-person hours: Monday 2:00–6:30 pm; Tuesday, Wednesday, and Thursday 2:00–6:00 pm. Virtual/flexible: 3 hours a week of admin/meetings. Travel required; time spent supporting practices with on-site presence at the following locations:

Locations, duties and on-site requirements may be adjusted to meet Fort Health operational needs.
In This Role, You Will Screen and assess patients for common mental health disorders, facilitate patient engagement and follow-up care.
Provide patient education about common mental health disorders and available treatment options.
Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, side effects, and effectiveness of treatment.
Provide brief behavioral interventions using evidence-based techniques (e.g., problem-solving treatment, motivational interviewing, behavioral activation).
Identify appropriate resources and coordinate referral processes to community resources when appropriate.
Participate in regularly scheduled caseload consultations with the psychiatric consultant and communicate treatment recommendations to the patient’s medical provider.
Collaborate with the collaborative care team to provide personalized treatment plans for every child and their family.
Maintain ongoing communication with families to drive the treatment plan forward.
Track patient follow-up and clinical outcomes using a registry and document progress and treatment recommendations in the electronic health record.
Develop and complete relapse prevention/self-management plans for patients nearing discharge from the caseload.
Participate in individual supervision with a psychologist to ensure support for success.
Access consultation and materials from the nonprofit partner to apply cutting-edge treatment protocols.
Expand clinical skills through the AIMS center and engage in ongoing training, conferences, and related opportunities.
What We Are Looking For Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), or other independent licensure eligible to practice.
Minimum 1 year of direct clinical experience with children, adolescents and their families (ages 4–24).
Experience and training in delivering brief psychosocial evidence-based treatments (e.g., CBT, problem-solving treatment, behavioral activation, motivational interviewing).
Experience with care coordination and preferably experience in a team-based healthcare setting.
Strong experience with screening for common mental health disorders and symptom assessment with children and adolescents; knowledge of differential diagnosis.
Strong engagement skills with parents and children, ability to develop treatment plans, and effective collaboration in a team setting.
Experience using measurement-based care tools (e.g., PHQ-9, GAD-7, SCARED).
Excellent verbal and written communication skills; experience providing telehealth.
Reliable, high-speed internet access (minimum 50 Mbps download and 10 Mbps upload).
