Schedule: Monday-Friday 8 AM - 5 PM | Split between two locations

What This Role Is All About
As a Behavioral Health Care Manager, you’ll be a key part of our collaborative care team, providing services as part of the Collaborative Care Model (CoCM). You’ll work closely with medical providers, psychiatric consultants, and other team members to support the mental and physical health of patients. You’ll help coordinate care for patients on an assigned patient caseload along with the medical provider. You will screen and assess patients for common mental health and substance use disorders and provide brief behavioral interventions using evidence-based techniques.

Responsibilities

Partner with medical providers to support patients mental and physical health

Screen for common mental health and substance use disorders

Provide brief behavioral interventions (behavioral activation, problem-solving treatments, motivational interviewing)

Track patient clinical outcomes using a registry

Educate patients on treatment options and wellness strategies

Refer patients to internal or external resources as needed

Document care plans and updates in the EHR

Collaborate with psychiatric consultants during weekly case reviews

Qualifications

Masters-level licensure in a behavioral health field (e.g., LCMHC, LCSW, LMFT)

Experience in mental health screening and treatment planning

Strong communication and teamwork skills

Comfortable working with patients in person and by phone

Familiarity with evidence-based interventions and psychopharmacology

Detail-oriented with solid documentation habits
