Program Description
Certified Community Behavioral Health Clinic (CCBHC) provides coordinated, comprehensive behavioral health care. The CCBHC serves anyone who requests care for mental health or substance use, regardless of ability to pay, diagnosis, or age. Services are provided in the Los Angeles vicinity, with virtual work opportunities. Our model of care includes comprehensive behavioral and physical health services and care coordination to address social drivers of health, including 24‑hour crisis services.

Compensation

Initial compensation: $69,971.00 – $86,055.00

Salary is commensurate with experience and internal equity considerations.

Salary may vary by location.

8% License Differential for LMFT, LCSW, LPCC, PsyD, and select Peer Certifications.

7.5% Bilingual Differential for qualified positions (Spanish approved).

Job Summary
The Behavioral Care Manager (BCM) coordinates mental and physical health care for an assigned patient caseload, collaborating with medical providers, psychiatric consultants, and other mental health providers. The BCM ensures effective communication among team members, supports medication management, delivers brief behavioral interventions, and facilitates referrals to evidence‑based psychosocial treatments.

Responsibilities and Duties

Support mental and physical health care for patients on an assigned caseload.

Coordinate care with the patient’s medical provider and other mental health providers.

Screen and assess patients for common mental health and substance abuse disorders.

Provide patient education on mental health and substance abuse disorders and treatment options.

Track treatment response and monitor patients (in person or telehealth).

Support psychotropic medication management, monitoring side effects and effectiveness.

Deliver brief behavioral interventions using evidence‑based techniques (behavioral activation, problem‑solving, motivational interviewing).

Facilitate referrals to evidence‑based psychosocial treatments.

Participate in caseload consultation with psychiatric consultant; communicate treatment recommendations.

Track patients’ follow‑up and clinical outcomes using a registry.

Document patient progress and treatment recommendations in EHR.

Facilitate treatment plan changes in consultation with medical provider and psychiatrist for patients not improving.

Practice self‑care and treat colleagues with grace.

Qualifications
To perform this job successfully, an individual must be able to perform each essential duty.

Master of Arts (M.A.) in Social Work or a closely related field (e.g., Marriage, Child and Family Counseling, or Psychology).

Three (3) years of supervised clinical experience.

Within 3 months of licensure or licensed according to California state law.

Strong leadership and crisis management skills.

If required, employee must pass an initial physical abilities test (PATS) and be certified in crisis intervention within first 30 days and annually.

Four (4) to five (5) years of clinical experience (preferred).

Training in domestic violence, physical/sexual child abuse, and suicidal trauma with current knowledge of state, county, and local law (preferred).

Self‑defense training to react/respond to harmful physical situations (preferred).

Knowledge of local county rules and regulations from Department of Social Services or related division (preferred).

Experience with culturally diverse populations (Highly desirable).

Speak, read, or write another language (Highly desirable).
