Responsibilities
The Advanced Practice Provider (APP) functions within the pediatric chronic care team ambulatory setting and within the neonatal ICU to manage the needs of the chronically ill patients. The APP provides care for chronically ill patients within the in‑patient NICU setting to help prepare the patient and guardians for transition to discharge. This includes, but is not limited to, assisting in preparation for discharge, coordinating home medical devices such as oxygen, feeding tubes, and monitors, and providing post‑discharge follow‑up care in the ambulatory setting, including telehealth. The APP works closely with outpatient testing groups and case management to ensure continuity of care and support for families transitioning to home. The APP also acts as a consultative resource for inpatient care teams and follows a strong foundation in evidence‑based practice, collaboration of care, and education.
Shift
Monday‑through Friday, equivalent to two 12‑hour shifts in the NICU and two 8‑hour shifts in the clinic setting.
Physical Requirements
Lifting, pushing, or pulling of objects up to 50 lbs, sitting for long periods of time.
Qualifications
Education: MSN for Nurse Practitioners or BS for Physician Assistants
Experience: 1 yr related experience
Certification: Advanced Practice Registered Nurse (APRN) with Neonatal Nurse Practitioner (NNP) certification or Physician Assistant‑Certified (PA‑C). Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) within 90 days of hire.
