AO (Adult Outpatient) Case Manager - Derby at Northeast Kingdom Human Services (NKHS)
The Adult Outpatient Case Manager responds to and develops initial crisis management and recovery plans for clients. The ideal candidate embraces excellent customer service, provides resource information, and connects clients to immediate access of services, thereby reducing barriers to treatment. In this dynamic role the Case Manager will also regularly provide early screening and intervention, case coordination, and urgent next‑day follow‑up and triage appointments.
Responsibilities & Scope
Provides immediate response upon referral receipt, offering excellent client service, resource information, expeditious access to services, identification and reduction of barriers, increased problem‑solving, and development of initial treatment‑plan goals.
Conducts early screening, intervention, and crisis counseling, including risk assessment and triaging immediate needs.
Performs case management, coordination, and referral services, maintains accurate clinical records and documentation, and actively participates in clinical team meetings to inform and coordinate services.
Consults and provides educational services to the community, agencies, professionals, and schools as required.
Supports urgent next‑day follow‑up and triage appointments.
