Minimum to Midpoint Pay Rate: $31.78 - $42.90 / hour
Summary
Lee Physician Group is seeking an experienced RN to join their new specialty located at the Regional Cancer Center. This RN will be the Ambulatory RN Care Coordinator for the plastic surgeon and must have excellent organizational skills and handle multiple priorities at one time.
The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient‑centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.
Responsibilities
Coordinate ambulatory care for breast surgery patients, including scheduling, education, and follow‑up.
Collaborate with physicians, nurses, and support staff to develop and implement individualized care plans.
Advocate for patients and ensure timely delivery of services while maintaining quality and cost‑effectiveness.
Educate patients and families on post‑operative care, medication management, and chronic disease management.
Maintain accurate and comprehensive patient records in accordance with regulatory and institutional standards.
Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.
Participate in interdisciplinary meetings and quality improvement initiatives.
Requirements
Educational Requirements: Nursing degree or diploma. Bachelor’s degree preferred.
Experience Requirements: Minimum 3 years of clinical nursing, with emphasis on patient education and chronic disease management. Experience in home health, skilled nursing, or medical‑surgical settings required. Care coordination experience preferred.
Licensure: Registered Nurse license in the State of Florida (Required).
