What is OhioRISE?
The Ohio Department of Medicaid (ODM) is committed to improving the health of Ohioans and strengthening communities and families through quality care. OhioRISE is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs.
NYAP is the Care Management Entity (CME) in Catchment Area C, delivering wraparound care coordination for children and youth enrolled in OhioRISE with moderate behavioral health needs, and supporting the system of care in Allen County and surrounding counties.
As a full-time CME Moderate Care Coordinator, you will cultivate flexible, family-focused, community-based services based on the High-Fidelity Wrap Around model, covering Allen County and overlapping into Auglaize County as needed. Under supervision of the Care Coordination Supervisor, the goal is to keep youth in homes, communities, and schools by assessing and delivering appropriate services and reducing out-of-home placements.
Responsibilities Cultivate flexible, family-focused, community-based services based on the High Fidelity Wrap Around model of care coordination
Develop and maintain the Wraparound Team, coordinate and lead team meetings
Coordinate and supervise implementation of the Plan of Care, including Transition and Crisis Plans with providers and community resources; update plans as needed
Ensure family support and stabilization during crises
Provide and document the initial and ongoing Life Domain Assessment
Maintain all service documentation, evaluation outcomes, and data as required
Provide services in a timely manner and in accordance with the Plan of Care and/or Crisis Plan
Utilize and monitor Flexible Funding and service coordination
Obtain weekly reports from subcontracted providers
Participate in after-hours on-call response
Attend program staff meetings, supervision and other required meetings
Participate in the Agency and Program CQI peer review process
Perform duties to reflect Agency policies and procedures and comply with regulatory standards
Meet Agency training requirements
Report all MUIs to Site Manager and Supervisor immediately
Other duties as assigned
Qualifications An MCC Care Coordinator may be licensed or unlicensed per rule 5160-27-01 of the Administrative Code
Complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM
Successfully complete skill and competency-based training to provide MCC
Experience providing community-based services to children and youth and their families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related fields; required level of education may vary:
three years with a high school diploma or equivalent; or
two years with an associate’s or bachelor’s degree; or
one year with a Master’s degree or higher
