Aftercare Case Manager – TFI Family Services
Join our dedicated team at TFI Family Services to support children and families who have achieved permanency through various pathways. This full‑time position is based in Wellington, KS.
What You’ll Need
Academic: A bachelor’s degree in Human Services or related field.
Valid driver’s license.
Willingness to learn and complete required training.
A Day in the Life
Focus on risk management and help families maintain stability during the aftercare period.
Provide crisis intervention and collaborate with public and private agencies offering supportive services.
Maintain accurate documentation and reports.
Coordinate community and educational services for families.
Meet with families in‑person to support reunification and resolve concerns.
