Essential Functions

Assumes responsibility for the effective coordination of patient/caregiver care.

Conducts nursing assessments and provides timely interventions, following established care plans and evaluating patient responses to progress toward goals.

Develops, implements, and evaluates plans of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions and education.

Performs emergency triage by telephone and home visits.

Facilitates patient and caregiver independence as much as possible.

Initiates and maintains liaison relationships with healthcare providers to coordinate multidisciplinary care, communicating pertinent information to team members.

Identifies and refers all patient/caregiver psychosocial needs to VIA Health Partners medical social worker as appropriate.

Provides necessary instruction to patients and caregivers, using teaching materials and documentation effectively.

Applies VIA Health Partners policies and protocols for hospice care, including medication management.

Maintains productive and professional relations with external agencies and healthcare providers.

Ensures work area is clean, secure, and well‑maintained; stores supplies safely.

Uses infection control principles and universal precautions appropriately.

Keeps clinical records current and submits all documentation per VIA Health Partners guidelines.

Manages medication profiles, teaching and documenting medication use.

Records time and mileage accurately.

Adheres to VIA Health Partners core values and employee handbook policies.

Keeps professional licensure and certifications current (RN license, CPR, CEUs).

Participates fully in staff meetings and related duties.

Qualifications

A diploma or associate’s degree in nursing (Bachelor’s preferred).

A current, active, unrestricted compact (multistate) RN license; CPR required, CHPN preferred.

Minimum two years of experience in direct care of patients with life‑limiting illnesses; experience in hospice and palliative care preferred.

Computer proficiency required.

Experience working in a pediatric-focused environment (Kids Path™ Nurse) with BCLS (CPR) training for adults and children/infants required.
