Job Summary
Responsible for managing the coordination of care across numerous agencies for our members. Requires an RN or LPN license and a minimum of 1 year caring for the elderly.

Position Summary
Educate, analyze, plan, coordinate and oversee service delivery. Evaluate member outcomes resulting from the care received.

Essential Functions

Identify member needs, goals and abilities

Ensure ongoing coordination

Prepare and submit a comprehensive, written service plan for each member

Implement certified ADvantage service plan

Monitor delivery of services

Identify high risk situations

Orient and educate members

Modify services to meet needs of members

Ensure confidentiality according to all relevant laws and regulations

Maintain adequate records

Justifies expenditures and monitors and contains cost of plan

Assist in implantation of in-service education for field employees

Skills

Current ADvantage Case Manager Certification Preferred

Excellent verbal, interpersonal and written communication skills

Excellent problem solving skills

Ability to use a computer and relevant data bases, especially for required reporting

Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs
