NJ Palliative Care Solutions, in strategic partnership with RWJBarnabas Health, is seeking skilled and compassionate Advanced Practice Nurses (APNs) to join its growing Palliative Care team for inpatient and outpatient opportunities across New Jersey.
This is a unique opportunity to become part of an integrated and mission-driven care model--offering both the autonomy of a community-focused organization and the resources of New Jersey's largest academic health system. While this position is employed through NJ Palliative Care Solutions, the selected candidate will operate alongside RWJBarnabas Health's clinical teams within RWJBarnabas Health facilities and care environments. Position Highlights
Deliver palliative care services across a variety of settings, including patients\' homes, skilled nursing facilities, assisted living facilities, and telehealth platforms
Participate in both inpatient and outpatient care , depending on site needs and preferences
Conduct comprehensive assessments, manage complex symptoms, and lead advanced care planning discussions
Collaborate closely with RWJBarnabas Health physicians, care managers, and community providers
Flexible schedule supported by a compassionate leadership team and robust administrative infrastructure
Qualifications
Active New Jersey APN license with prescriptive authority
Valid CDS and DEA registrations
Minimum of 3 years' clinical experience in palliative care, geriatrics, family medicine, or oncology preferred
Strong clinical judgment, communication skills, and a commitment to whole-person care
Reliable transportation and a valid driver\'s license are required

The anticipated salary range for this position if hired to work is $125,000 to $140,000 per year.
