Overview This is responsible and specialized case management work in assessing, planning, linking, monitoring, and advocating for a caseload of adults with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through the development of the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with their families or guardians, community resources and advocating for clients by linking them with appropriate services.
Responsibilities Provide case management/therapeutic mentoring services to an assigned group of clients
Meet with assigned clients on a weekly basis or as indicated by treatment planning
Act as a liaison and consult with school systems, community agencies, and families to maintain coordination of the treatment process
Advocate for families' rights and needs
Refer adults and families for additional services as identified
Meet expectations regarding documentation deadlines and utilization trends
Preparation of social skill building activities for each client
Assist with assessing safety issues that may have an impact on the service
Provide transportation, respite, and tutoring services as assigned
Qualifications Bachelor's degree in human services or related field
Six (6) months experience required; one (1) year of experience preferred
Valid driver's license, with a clean driving record
