Adult Case Manager – Pecan Valley Centers for Behavioral and Developmental Healthcare
Provides ongoing case management and rehabilitation services for adults with mental illness. Coordinates services, conducts assessments, develops recovery plans, and communicates with clients, families, and community agencies to facilitate comprehensive treatment.
Job Summary
Coordinate and provide mental health case management services to adult clients, applying trauma‑informed care principles. Plan and implement individualized recovery plans, conduct effective assessments, and maintain professional relationships with community partners.
Duties & Essential Job Functions
Maintain knowledge of TRR services and UM guidelines for all locations.
Accurately complete a uniform assessment to identify needs and strengths.
Utilize an electronic health record system for documentation across all service processes.
Meet productivity and performance standards set by the agency.
Maintain a flexible work schedule with effective time management.
Travel between locations as assigned.
Participate in weekly/monthly team meetings.
Assist in data collection activities and peer review processes.
Perform other duties assigned by supervisor(s).
Knowledge, Skills, and Abilities
Working knowledge of mental health and substance abuse services.
Strong computer operating skills.
Excellent verbal and written communication skills.
Ability to work under pressure and meet deadlines.
Capacity for flexibility in an evolving service delivery system.
Education & Experience
Required: Bachelor’s degree in a human services field. Preferred: Master’s degree. Experience in a social, behavioral, or human services occupation is preferred.
Licenses & Certifications
QMHP-CS status is necessary. Must have a valid Texas driver’s license, a clean driving record, and required automobile liability insurance.
Physical Requirements
Ability to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for extended periods, reach overhead, and walk over uneven terrain.
