Position Details
Position: Acute Care Coordinator

Location: Las Vegas, Nevada – Onsite (This role is not eligible for Remote or Hybrid)

Shift: Sunday, Monday, Friday & Saturday

10‑hour shift: 8:30am–7pm

Off Days: Tuesday, Wednesday & Thursday

Starting Salary Range: $23 per hour

The starting salary range for this role is a reasonable estimate, with a goal to pay a competitive market salary focusing near the median of our pay ranges. However, final offers for all positions will be based on several factors such as experience level, education and certification, skills, and internal pay equity.

What’s the role?
The Acute Care Coordinator plays a vital role in facilitating and coordinating healthcare services for individuals within the community. The purpose of this role is to connect patients with the appropriate resources, coordinate care plans, and ensure seamless transitions between healthcare providers to promote optimal patient outcomes and enhance community health. The ACC position works in the field, conducting assessments at various locations such as hospitals, clinics, and community settings.

What you’ll do:

Coordinates the discharge planning and social service program, identifies patients needing psychiatric/mental health services after discharge

Ensures smooth transition and comprehensive discharge plans for their patient and their family.

Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care.

Coordinates with patient the activities to meet patient’s goals

Documentation of all interventions within the code and services provided.

Exceptional leadership and team management skills, with the ability to motivate and engage a diverse team of healthcare professionals.

Discharge planning psychiatric and social determinants of health related

Strategic thinking and problem‑solving abilities to drive operational efficiency and quality improvement.

Commitment to patient‑centered care and continuous quality improvement.

Ability to build and maintain relationships with community organizations and stakeholders.

Determine and coordinate referrals to internal programs, external treatment providers, or community resources.

Collaborate with multidisciplinary teams: Work closely with other healthcare professionals, including psychiatrists, therapists, and social workers, to ensure comprehensive care for clients.

Document assessments: Maintain accurate and detailed documentation of assessment findings, treatment plans, and progress notes in compliance with company policies

Maintain ethical standards: Adhere to professional and ethical guidelines and maintain confidentiality of client information.

This position may be required to drive between different hospitals in the greater Las Vegas Area. As such, access to reliable transportation is required which can safely get the employee from one hospital to another in under an hour.

Advocating for patients' needs and rights within the healthcare system and community resources.

Documenting patient interactions, care plans, and progress notes in electronic health records or care management systems.

Additional duties as assigned.

What you’ll bring:

Comprehensive knowledge of healthcare systems, community resources, and social services available to support patient care.

Strong interpersonal and communication skills to establish rapport with patients, families, and healthcare providers.

Excellent organizational and time management skills to manage multiple patients and coordinate care effectively.

Ability to assess patient needs, develop individualized care plans, and evaluate outcomes.

Proficiency in using electronic health records and care management software.

Understanding of privacy regulations and compliance requirements related to patient information and data security.

Compassionate and patient‑centered approach to care, with a commitment to advocating for vulnerable populations.

Problems‑solving and critical thinking skills to address barriers to care and resolve complex situations.

Cultural competence and sensitivity to diverse populations and their unique healthcare needs.

Ability to work collaboratively in a team and maintain positive relationships with healthcare professionals and community partners.

What you’ll need to start:

Typically, a minimum of 1 year of experience in healthcare coordination, case management, or related roles is required, preferably in community‑based settings.

Bachelor's degree in healthcare administration, social work, nursing, or a related field. Additional certifications or specialized training in care coordination or case management are desirable.

CPR/First Aid

Must pass drug screening and background check in order to start employment with Novum Health.

Must have independent transportation vehicle to visit multiple locations.

Must be willing to receive vaccinations in accordance with hospital and state requirements.
