What’s the role?
The Acute Care Coordinator facilitates and coordinates healthcare services for individuals within the community. The purpose is to connect patients with the appropriate resources, coordinate care plans, and ensure seamless transitions between healthcare providers to promote optimal patient outcomes.
Responsibilities
Coordinate discharge planning and social service programs, identifying patients needing psychiatric/mental health services after discharge.
Ensure smooth transition and comprehensive discharge plans for patients and families.
Schedule appointments for patients with multiple healthcare providers to ensure continuity of care.
Coordinate activities with patients to meet their goals.
Document all interventions within the code and services provided.
Provide exceptional leadership and team management skills, motivating and engaging a diverse team of healthcare professionals.
Engage in psychiatric and social determinants of health related discharge planning.
Apply strategic thinking and problem‑solving abilities to drive operational efficiency and quality improvement.
Commit to patient‑centered care and continuous quality improvement.
Build and maintain relationships with community organizations and stakeholders.
Determine and coordinate referrals to internal programs, external treatment providers, or community resources.
Collaborate with multidisciplinary teams, working closely with psychiatrists, therapists, and social workers.
Maintain accurate and detailed documentation of assessment findings, treatment plans, and progress notes in compliance with company policies.
Adhere to professional and ethical guidelines and maintain confidentiality of client information.
Drive travel between different hospitals in the greater Las Vegas Area; reliable transportation required to move between hospitals in under an hour.
Advocate for patients’ needs and rights within the healthcare system and community resources.
Document patient interactions, care plans, and progress notes in electronic health records or care management systems.
Complete additional duties as assigned.

Must‑Start Requirements
Minimum of 1 year of experience in healthcare coordination, case management, or related roles, preferably in community‑based settings.
Bachelor’s degree in healthcare administration, social work, nursing, or related field. Additional certifications or specialized training in coordination or case management desirable.
CPR/First Aid certification.
Pass a drug screening and background check.
Independent transportation vehicle to visit multiple locations.
Willingness to receive vaccinations according to hospital and state requirements.
