Job Summary The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery based and trauma-informed perspective. Care Coordinators/Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate clinical skills by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Care Coordinators/Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. They participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. They may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. They may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, they work under the supervision of an LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.
On-call responsibilities may be required depending upon unit protocols. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.
Regular, predictable attendance is considered an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule. Integral Care employees strive to maintain a trauma sensitive workplace and to foster a supportive and welcoming environment of care.
Education Required: Bachelor’s degree from an accredited college or university with a major in Human/Behavioral/Social Services or related field. Master’s degree preferred.
License Current and valid driver's license and a clean driving record.
Experience Bilingual preferred.
Hours Full-Time
Workdays Monday - Friday
Workhours 8:00 AM - 5:00 PM
Salary $24.18- $25.39 per hour based on education plus an additional 10% crisis stipend.
