The Access Case Manager coordinates services for children and families that are experiencing mental health crises. The case manager works with families to develop treatment plans and implement therapeutic interventions to support clients short‑term while determining what ongoing services would support them. The case manager will hold cases including those who are reintegrating into their family home while they are in an access placement, those who utilize the EPIC program, and following intake appointments while waiting for ongoing services. The access case manager will be trained to support the EPIC program including but not limited to being able to admit and discharge clients from the EPIC episode in myavatar, responding to referrals from the mobile crisis team, checking clients into and out of the EPIC day program, and utilizing the EPIC calendar on Outlook to track the utilization of the EPIC space. The case manager is responsible to coordinate services, collaborate with other service providers, and maintain appropriate documentation of services provided including partnering with the Mobile Crisis Team, School-Based Therapeutic Services Director, and School-Based Clinical Services Director, to identify clients who are in crisis and are appropriate for Access services. Services offered by this position are across settings, including office, home, community, residential homes, and school. This position has a strong connection with mobile crisis team.
Responsibilities

Determine ongoing service needs and coordinate appropriate supports.
Hold cases for families reintegrating into their family homes while in an access placement, EPIC program participants, and clients awaiting intake appointments.
Admit and discharge clients from EPIC episodes in myavatar, respond to referrals from the Mobile Crisis Team, check clients into and out of the EPIC day program, and use the EPIC calendar on Outlook to track EPIC space utilization.
