Overview AAA Care Coordinator, Cottonwood — Northern Arizona Council of Governments
$22.67/hr, 40 hrs/wk, 52 wks/yr
This position is on-site. A hybrid work model may be considered after successful completion of the probationary period, based on program needs and performance.
Position Summary In compliance with Federal Law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire.
Responsibilities Conduct home visits and interviews to assess eligibility for program services
Identify program services that will best meet the needs of clients
Maintain and update electronic client files to ensure accurate record keeping and program compliance
Provide resource referrals and advocate for appropriate support services outside the agency to ensure access to comprehensive services
Monitor services to clients to ensure high quality service delivery and program compliance
Respond to public inquiries for information regarding program services
Perform other related duties as assigned
Education And/Or Experience Bachelor’s Degree from an accredited college or university in social work, health services, human services, or related field; and two years’ experience in a social service field; OR an equivalent combination of education, training, and experience that demonstrates the ability to perform the duties of the position.

Travel Required Less than 5% to up to 25% as needed; other ranges may apply based on program needs.
Physical Requirements & Work Environment Physical Activity : Frequent listening, talking, walking, sitting, stooping, reaching, finger use, grasping, and repetitive motion. Occasional standing, climbing, balancing, kneeling, crouching, pushing, pulling, lifting.
Physical exertion : Sedentary to light; work involves exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently.
Visual requirements : Visual acuity to operate vehicles or equipment.
Working conditions : Occasional exposure to weather conditions, potential physical harm, chemicals, noxious odors, and infectious disease.
