Job Description
As a valued member of the Bluebonnet Trails Community Services Youth and Family services team, this professional position conducts a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, oversee implementation of the plan, identify providers of services or family‑based resources, facilitate child and family team meetings, and monitor all services authorized for a child’s care.
The Wraparound Facilitator assures that care is delivered in a manner consistent with strength‑based, family‑driven, youth‑guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.
Responsibilities
Conduct comprehensive assessment of youth and families.
Develop and implement plan of care in partnership with team members.
Identify service providers or family‑based resources.
Facilitate child and family team meetings.
Monitor services authorized for a child’s care.
Deliver care consistent with strength‑based and culturally competent values.
Consult and educate providers regarding model values and principles.
Monitor progress toward treatment goals and record evaluation data.
Qualifications
Bachelor’s degree in a social, behavioral, or human service field or equivalent coursework.
Knowledge of effects of mental illness.
Valid Texas Driver’s License and approval to maintain active driving status.
Experience in mental health assessment, crisis intervention, and screening preferred.
Master’s degree in psychology, social work, or related field preferred.
Bilingual preferred.
Location and Compensation
Locations: Fayette County and Lee County
Hourly Rate: $22.22 – $27.97 per hour, plus applicable differentials such as $0.75/hr Bilingual, $1.00/hr Human Services Master’s degree, and other listed differential amounts. Monthly Service Delivery incentive: $200.
