In this role, you will use your clinical expertise to conduct Start of Care (SOC) assessments, collaborate closely with physicians, and administer critical treatments including IV therapies. Working within a small, targeted territory and with flexible weekend hours, you'll have autonomy in your work and access to strong support systems. Accountabilities:

  • Perform Start of Care (SOC) admissions and complete OASIS documentation for new patients.
  • Conduct follow-ups and evaluation visits on weekends in the assigned territory.
  • Monitor and document patient conditions and coordinate care plans with physicians and internal teams.
  • Administer IV medications such as antibiotics and diuretics in accordance with protocols.
  • Provide hands-on patient care including wound treatment and medication reconciliation.
  • Maintain accurate and timely documentation of all care interventions and outcomes.
  • Collaborate with an interdisciplinary team to ensure the highest standards of care delivery.
  • Active and unrestricted Registered Nurse license in California.
  • Current BLS/CPR certification and valid CA driver’s license.
  • Experience with Start of Care (SOC) and OASIS documentation preferred.
  • Background in high-acuity patient care and case management is strongly desired.
  • Proficiency in IV therapy initiation and medication administration is a plus.
  • Strong critical thinking, organizational, and interpersonal communication skills.
  • Self-directed, empathetic, and collaborative team player with leadership potential.
