Midtown Oaks Post Acute is looking for a dedicated and compassionate Telehealth Physical Therapist to join our dynamic rehab team! Position Highlights:

• Provide virtual physical therapy evaluations and treatments

• Develop and implement individualized care plans

• Collaborate closely with on-site interdisciplinary team members

• Monitor patient progress and adjust treatment plans as needed

• Educate patients and caregivers on therapeutic exercises and safety

What We’re Looking For:

• Active California Physical Therapist license

• Strong clinical assessment and documentation skills

• Experience in skilled nursing or post-acute care preferred

• Comfortable utilizing telehealth platforms and technology

• Compassionate, motivated, and team-oriented

• Ability to work effectively in a fast-paced environment
