Position Summary

The Hospice QA Manager ensures compliance with Medicare Conditions of Participation, ACHC standards, and internal policies. This role manages clinical workflow in HCHB, coordinates Quality Assurance and Performance Improvement (QAPI) programs, and supports the Director of Clinical Services (DCS) in maintaining regulatory excellence.

Key Responsibilities

  • Review and manage hospice clinical workflow tasks in Homecare Homebase (HCHB) for all disciplines
  • Verify admissions, Plans of Care, HIS documentation, CTIs, physician orders, and recertifications
  • Coordinate QAPI program: compile data on patient care, safety, infection control, outcomes, and satisfaction
  • Ensure compliance with state, federal, and accreditation guidelines
  • Provide feedback on documentation trends and assist with chart audits
  • Maintain professional knowledge through continuing education
  • Communicate with physicians regarding patient needs and obtain orders as required

Qualifications

  • Active California RN License (Required)
  • 2+ years hospice quality assurance experience (Required)
  • Strong knowledge of Medicare regulations, ACHC standards, and hospice compliance
  • Proficient in Homecare Homebase (HCHB) and computer systems
  • Excellent communication, coordination, and organizational skills
  • Valid driver’s license and insured vehicle
