Our client, a mission-driven nonprofit organization focused on supporting individuals with ALS, is seeking a Manager, Care Services based in Baltimore, MD, and surrounding counties. This is a remote role with significant travel throughout the Baltimore metropolitan area. The Care Services Manager will play a critical role in ensuring high-quality, consistent care and support services are available to individuals with ALS and their families.

Responsibilities
  • Conduct care consultations and needs assessments for individuals living with ALS, their caregivers, and families, particularly in complex situations.
  • Manage a portfolio of clients to ensure their needs are met through available programs and services.
  • Provide information on services, programs, and resources, including oversight of specific program areas.
  • Coordinate referrals to healthcare providers and community-based organizations.
  • Evaluate and support community-based support groups to meet the needs of the ALS population.
  • Collaborate with internal leadership to plan and evaluate program effectiveness.
  • Maintain regular engagement with individuals with ALS and/or their caregivers (at least quarterly).
  • Accurately document all interactions with clients, caregivers, and healthcare professionals.
  • Deliver public and professional education regarding ALS, care options, and available resources.
  • Serve as a liaison with multidisciplinary ALS clinic teams and maintain strong relationships with clinical partners.
  • Participate in relevant workshops, conferences, and continuing education opportunities.
  • Provide mentorship and support to newly onboarded team members.
  • Bachelor’s degree in social work, case management, health administration, allied/public health, nursing, or a closely related field (or equivalent professional experience).
  • At least 4 years of relevant experience, or an equivalent combination of education and training.
  • Strong skills in critical thinking, problem-solving, program planning, performance monitoring, and organizational management.
  • Familiarity with ALS or other progressive neurodegenerative diseases is beneficial.
  • Working knowledge of Medicare, Medicaid, patient assessment, care planning, and case management is preferred.

  • Annual salary range: $50,649 – $79,346, based on location, experience, and related factors.
