Job Description

Summary

This position is available for licensed Registered Nurses (RN). The Lab Nurse is responsible for reviewing, organizing, and managing patient lab orders and results, ensuring timely documentation, communication, and follow-up. Under the direction of nursing leadership, the Lab Nurse performs designed clinical, technical, and clerical tasks related to lab management, pharmacy coordination, and patient care workflows. This role requires strong critical-thinking skills, effective communication, and proficiency in clinical software systems to support safe and accurate patient care.

Hourly Rate: $34/hr - $42/hr (DOE)

Schedule: (Remote) Monday - Friday, 8:30am - 5:00pm CST specifically

Location: Must reside in the U.S. and work the above schedule but is not a requirement to reside in a CST state

Essential Duties and Responsibilities

Serve as the primary interface to the Agency’s referral sources and maintain responsibility for receiving referrals and having a comprehensive understanding of all clinical and service needs of the Agency’s patients. Work collaboratively with all departments to ensure optimal patient care and timely service by our clinical staff.

  • Assume responsibility for daily operations of the Palliative Department.
  • Follow all patients through the referral process with the goal of converting to a consult.
  • Oversee communication to referral sources regarding the status of referrals and solicit their help in getting the appropriate documentation to admit.
  • Communicate with Sales teams about the status of referral.
  • Assume responsibility for ensuring task screens in HCHB are completed.
  • Responsible for annual evaluations for all direct reports.
  • Manage time off requests for all direct reports.
  • Responsible for communication with Sales team as required.
  • Assist leadership with identifying program needs for focused development.
  • Develop, implement, and maintain Intake process and policies and procedures congruent with agency goals.
  • Assist clinical personnel in care coordination of patient services.
  • Serve as a liaison between the company, referral sources, patients, and agency personnel.
  • Collaboration with Sales, Home Health, Journey, and Hospice teams.
  • May interact with patients and patients’ families to coordinate visits.
  • Manage Palliative workflows and non-admits
  • Conduct chart reviews and high acuity patients

  • High School diploma/GED
  • 2 years of Hospice and/or Home Health experience preferred
  • Medical Terminology knowledge preferred
  • Active listening skills
  • Customer service skills
  • Possess patience, dependability, good and sound judgement, and ability to manage concurrent demands
  • Proficient in Microsoft office suite including teams 
  • Detail Oriented with use of time management techniques and tools
