Job Description

Major Responsibilities:

Conduct Plan of Care Review to ensure documentation is complete, comprehensive, and individualized for the patient, while meeting regulatory and compliance requirements.

Work to submit Plan of Care documentation in an accurate and timely manner.

Exercise best practice, critical thinking and clinical judgment as it applies to Plan of Care and clinical documentation.

Develop and sustain relationships that enhance effective coordination of care for our patients.
 

Internal department Care & Treatment Documentation Coordination:

Collaborate with clinicians, team leaders, Clinical Managers, and leadership regarding treatment provision and changes to Plan of Care documentation.

Collaborate with other Advocate Home Care Administrative Departments to effectively and efficiently release a complete and comprehensive Plan of Care document.

Maintain knowledge of current regulatory and compliance home care guidelines and admission criteria.

Participate in multi-disciplinary patient care conferences, as needed.

Attend regularly scheduled team meetings.

Other Duties as assigned.

Licensure:
Nurse, Registered (RN) • Illinois RN License

Education/Experience Required:
• Registered Nurse • 3 years combined adult med/surg. experience • 2 years homecare experience as a RN • Some introduction to intake coordination or care coordination with a homecare company preferred

Knowledge, Skills & Abilities Required:
• Exceptional interpersonal skills - able to work well with diverse personality types in a fast-paced environment - internal & external customers • Strong analytical skills • Excellent communication skills • Strong knowledge of home care services • Good working knowledge of home care insurance benefits –Medicare (Part A & Part B), Medicaid, Managed Care • Familiarity with Epic operating systems and Microsoft Office products preferred

Physical Requirements and Working Conditions:
Must maintain internet access connectivity on a regular basis to complete the functions of the job • Adhere to organization Security measures and HIPPA Standards • Ability to commute to physical office location on an as needed basis to procure technology/equipment needed to complete requirements of the job
