Whole-Person Integrated Care
NorthPoint offers a continuum of social and health services that integrates medical, dental, and behavioral health with human services to improve quality of life. We strive to serve our community by addressing the physical, behavioral, social, and spiritual aspects of health in culturally responsive and trauma-informed ways.
Building Community Well-Being
A community's sense of well-being includes social, economic, environmental, cultural, and political conditions identified by the members of a community as essential for them to "flourish and fulfill their potential." To build and nurture community well-being, we must understand that population we serve to improve their health, safety, and access to the services they need.
Health Equity

General Job Statement
The Case Manager will adopt a Growth Focused Case Management framework to engage youth and families in achieving stability, educational success, and positive community connections. This innovative approach emphasizes empowerment and utilizes strengths-based strategies, fostering collaborative coordination of services tailored to the unique needs of each participant.
By leveraging both internal and external partnerships, the Case Manager will provide comprehensive service coordination and vital information to youth and their families on a range of social and economic issues. This includes legal assistance, social services, medical referrals, educational resources, housing solutions, employment opportunities, and transportation support.
Minimum Qualifications
High School Diploma/GED with 7 years' experience working with a human services organization or Associate degree in human services or related field with three years' experience working with a human services agency or organization in case management or care coordination, required. Bachelor's degree in human services or related degree, preferred.
Minimum 1-2 years experience in case management or youth mentorship within non-profit, educational, or justice-involved settings.
Ability to develop and maintain rapport with diverse groups of youth that encourages empowerment, self-sufficiency, and non-dependence.
Demonstrated ability to perform programmatic and case management functions.
Experience with justice-involved youth preferred.
Understanding of the impact of trauma on youth development.
Experience facilitating group activities.
Must be able to interact effectively with youth and their caregivers of various ages, ethnic, cultural backgrounds, and economic status.
Must have knowledge of community, educational and economic resources, and social services throughout Hennepin County.
Highly developed organizational skills and comfortable taking initiative.
Excellent oral, written and group communication skills.
Must have a valid driver's license, reliable transportation and the ability to meet NorthPoint's driving standards. Must be able to drive NorthPoint's passenger van to safely transport youth to and from programming.
Must be proficient in MS Word, MS Excel, and computer client tracking databases.
Work Environment
Work is performed in a standard office environment. Work involves regular guest interaction and extensive computer use (6-8 hours daily), requiring hand-eye coordination for standard office equipment.
