Base pay range $50.00/yr - $60.00/yr
Position Type: Full-Time

Schedule: Monday–Friday, 9:00 AM to 5:00 PM, with flexibility for late afternoon sessions or service delivery as needed.

Overview

Are you a Registered Nurse with a passion for youth mental health and community impact? We're seeking a Youth Clinical Care Coordinator who thrives in dynamic environments—whether embedded in a school, delivering care in the community, or supporting families where they need it most. This position is ideal for a compassionate, organized, and proactive professional eager to make a lasting difference in the lives of youth.

The role focuses on comprehensive care coordination and supportive clinical services, ensuring that young people and their families have access to essential health, behavioral, and social supports. Working closely with schools, families, healthcare providers, and community resources, the Youth Clinical Care Coordinator serves as a trusted ally and advocate for youth well-being.

Key Responsibilities

Care Coordination & Clinical Support

Assess youth and family needs, strengths, and goals to develop individualized care plans
Facilitate connections to mental health services, physical healthcare, education support, and social resources
Offer direct clinical interventions, such as informal counseling, crisis response, and psychoeducation
Conduct regular check-ins and monitor progress, adjusting plans as needed to enhance outcomes
Help families navigate healthcare systems, insurance benefits, and other service networks

Collaboration & Advocacy

Work alongside multidisciplinary teams including therapists, educators, case managers, and school staff
Advocate for youth and families within systems to ensure access and equity
Participate in school-based and community meetings to contribute to youth support planning
Provide services outside the school setting when needed, including in-home or neighborhood environments

Documentation & Accountability

Maintain accurate, confidential records in accordance with HIPAA and agency policies
Ensure all required documentation, such as treatment plans, are signed off appropriately
Submit timely caseload inventory logs; track care outcomes and compliance measures
Document all care coordination activities and clinical interventions in the electronic health record (EHR)

Crisis Response & Outreach

Respond to youth or family crises with immediate support and intervention
Participate in outreach efforts to increase awareness of services and strengthen community relationships
Promote trauma-informed and youth-centered strategies to build trust and engagement

Qualifications

Active RN license in the District of Columbia
Experience working with youth, adolescents, and families in behavioral health, school-based, or community settings
Knowledge of mental health diagnoses, treatment planning, and trauma-informed care
Strong communication, documentation, and relationship-building skills
Proficiency in electronic health record (EHR) systems
Availability to work flexible hours, including late afternoons and off-site care when needed
NPI registration and provider affiliation must be completed prior to employment
