About the Job The Care Manager conducts youth‑centered youth/family team meetings to determine client’s strengths and needs and then what services are needed to address those needs. Provides general client services and case management in a Montgomery County Public School
Some of the exciting tasks you will do as a Care Manager include but are not limited to: Accepts referrals for social services and completes initial intakes
Schedules and conducts Youth/Family Team meeting to gather information for YFSP making effort to include all family support available
Consults with county/community/agencies to identify and develop potential resources for client assistance
Designs intervention strategy w/youth’s family and/or support team match appropriate service to the youth and/or family’s individual needs
Provides feedback on referrals/intake to SCYS team to develop strategy for youth/family team meeting
Connect referrals to outside service providers and resources as needed
Provides case management and monitoring services to youth/families
Outreach and network with youth, families, communities, schools, and other community agencies
Provides on‑going social services (appropriate assistance during crises, may provide supportive counseling, food, clothing, housing, employment, transportation, financial assistance)
Performs accurate record‑keeping including intakes, developing, reviewing and updating the Youth/Family Service Plan, contact notes and electronic database entry
Qualifications and Skills: Bachelor’s degree in psychology, social work, sociology, criminal justice, family science or other human service related field.
1‑3 years’ experience
Experience in working with diverse populations; and skilled at collaborating with multiple agencies
Bi‑lingual in English and Spanish preferred
Clear, effective written and verbal communication
Working Conditions: Must be able to stand or sit for prolonged periods of time
Must be able to handle heavy workload and multiple job related pressures
Ability to drive from branch site to branch site and to Association Services Office
Working at home some evenings is necessary to complete assignments
