Position Summary
The YHDP TLP Case Manager will provide direct support to youth in the program by assessing their needs, developing individualized service plans, and helping them achieve independent living goals. The position involves case management, crisis intervention, life skills training, and coordination with other community resources.
Key Responsibilities
Case Management:
- Develop and implement individualized service plans, outlining clear goals and actionable steps toward independent living.
- Monitor progress and adjust plans as needed to ensure goal attainment.
- Provide direct support in areas such as education, employment, health, and financial management.
Life Skills Coaching:
- Deliver practical instruction and guidance in essential life skills, including:
- Financial literacy (budgeting, saving, bill management)
- Employment readiness and career planning
- Household management (meal preparation, cleaning, basic maintenance)
- Effective communication and problem-solving strategies
- Facilitate group sessions and workshops to reinforce these skills in a supportive setting.
Crisis Intervention:
- Respond to urgent situations impacting youth stability, such as housing concerns, health crises, or interpersonal conflicts.
- Coordinate access to appropriate support services and resources for immediate and long-term solutions.
Resource Navigation:
- Assist youth in identifying and utilizing community resources, including education, healthcare, employment, transportation, and housing.
- Establish and maintain partnerships with local service providers and community organizations to expand available resources.
- Support youth in connecting with legal, social, and health-related services as needed.
Monitoring & Documentation:
- Maintain accurate, timely records, including case notes, progress tracking, and service documentation.
- Participate in data collection, reporting, and evaluation activities.
- Ensure compliance with privacy and confidentiality regulations, including HIPAA.
- Contribute to program improvement by providing feedback and recommendations.
Advocacy:
- Support youth in understanding their available resources and options.
- Assist in navigating systems and making informed decisions that align with their personal goals.
- Serve as a liaison to connect youth with supportive services that promote overall well-being and self-sufficiency.
Collaboration:
- Work closely with internal teams and external partners to provide coordinated support.
- Participate in team meetings, case discussions, and ongoing professional development.
- Foster a team approach to service delivery that prioritizes effective communication and shared responsibility.
Qualifications
Knowledge, Skills, and Abilities:
- Proficiency in Microsoft Office Suite and standard documentation practices.
- Strong communication and relationship-building skills.
- Ability to effectively engage with youth facing complex life challenges.
- Competence in managing crisis situations and applying sound problem-solving strategies.
- Ability to work in a dynamic, fast-paced environment with evolving priorities.
- Familiarity with community resources, social service systems, and supportive programs for youth.
- Capability to manage a caseload independently while meeting performance expectations.
Certifications (Preferred):
- CPR/First Aid Certification.
- Valid driver's license and reliable transportation (if travel is required).
Education & Experience:
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field preferred; equivalent relevant experience will be considered.
- 2-5 years of experience providing direct support to youth or individuals in similar service environments.
- Experience with case management, crisis intervention, and service coordination.
