Weekend (every other) Triage Hospice Nurse - Registered Nurse (RN) We are seeking an RN to work every other weekend from 8:00?am to 5:30?pm on both Saturday and Sunday. This could be a 30?hour/week role with additional hours seeing patients.

Responsibilities Provide care and services for a group of hospice patients
Visit patients where they reside, including private homes, assisted living facilities, and hospitals
Complete hospice admissions and patient visits as needed
Answer calls from patients, families, and customers
Make nursing visits for symptom management, deaths, and admissions as needed
Work independently and on your own on a day?to?day basis with activities including phone, email, and written communication
Qualifications Current RN licensure in good standing in the state(s) of practice with one year of experience under license
Admissions and on?call experience preferred
Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
Can work independently and on your own day?to?day with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations
Time management skills to manage visits, documentation, and meetings (schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn an electronic medical record system
Willingly drive with reliable transportation, valid drivers license, and auto insurance to travel across the service area
