Overview Job Description - Virtual Care Coordinator - Telemedicine (Mon - Fri, 2pm - 10pm)
The Virtual Care Coordinator supports operations of the Telehealth Command Center at Children’s National Hospital. Provides virtual surveillance for patients with critical congenital or acquired heart disease in the CICU at Children’s National Hospital, as well as others in remote institutions nationally or internationally. Monitors patient vital signs and trends from multiple monitoring devices; alerts bedside teams of concerning trends when appropriate. Coordinates nursing care and services for patients and family members in specific healthcare encounters by telephonic, electronic, in person, and/or by mail. Consults with physicians and other healthcare providers to resolve health concerns. Collaborates with all members of the health care team in planning and delivering patient care. Demonstrates evidence-based practice and leads activities of the patient care delivery team for specific patient populations to ensure continuity of care."
