Overview Unlicensed Military Case Manager (Inpatient) (Full Time) at Aurora San Diego Hospital.
Base pay range $72,800.00/yr - $76,960.00/yr
Responsibilities Provide quality case management and psychotherapeutic services to adult military patients (age ~18 and up) and their families as part of an interdisciplinary treatment team.
Act as liaison between the facility, military providers, and various commands to assure delivery of quality treatment.
Conduct psychological assessments to document history and identify treatment focus by interviewing the patient and family.
Provide group and family process-oriented therapies as well as educational and didactic groups for patients and family members using various treatment modalities.
Develop and coordinate individualized discharge plans for patients, utilizing treatment team and chart information to determine aftercare needs; identify and assess family or community resources.
Attend regular treatment planning meetings and communicate state or local agency legal requirements for case management with the interdisciplinary team.
Report to community/state agencies on patient issues as required and represent the facility in legal proceedings when needed.
Provide backup support for incoming assessments and referral calls.
Comply with facility safety, infection control, and security programs; support internal and external customer service standards.
Establish contact with potential referral sources and participate in unit/program orientation to stay current on services and changes impacting referrals.
Maintain development activity documents and records; assist supervisor with other marketing activities.
Qualifications Master7s Degree in Social Work or another Board of Behavioral Sciences accredited discipline.
Knowledge of acute adult psychological disorders and advanced principles of abnormal psychology as applied to adults.
BLS (Basic Life Support) Certification for Healthcare Providers per American Heart Association guidelines.
