Transitional Housing Case Manager

If youre passionate about making a difference and want to empower survivors to thrive beyond crisis, wed love to have you on our team.The Transitional Housing Case Manager provides trauma-informed advocacy and case management for survivors in Avalons Transitional Housing Program. This role helps survivors navigate housing barriers, access community supports, and develop long-term stability plans that promote safety, self-sufficiency, and empowerment.

Housing Advocacy

  • Conduct comprehensive assessments and intakes for survivors entering the Transitional Housing Program.
  • Collaborate with survivors to develop individualized Housing Stability Plans, connecting them to community, housing, and financial resources.
  • Identify safe and appropriate housing options and support survivors through the move-in process.
  • Empower survivors by focusing on their strengths, needs, and goals for permanent housing and independence.

Direct Services & Case Management

  • Provide trauma-informed case management and goal planning to promote survivor stability and self-sufficiency.
  • Serve as a liaison with local agencies, service providers, and community partners to coordinate wraparound support.
  • Offer comprehensive advocacy services, including:
    • Legal, medical, and systems advocacy
    • Crisis intervention and safety planning
    • Emotional and financial advocacy
    • Employment and housing support
    • Material assistance and transportation
  • Maintain accurate, confidential documentation of services, client progress, and program expenses (rent, utilities, etc.).
  • Work collaboratively with Avalon Center staff to ensure client needs are met holistically.

Community Collaboration

  • Build strong relationships with landlords, utility companies, and local agencies to expand housing opportunities for survivors.
  • Act as a community liaison to raise awareness and strengthen coordinated responses to domestic violence and sexual assault.
  • Represent Avalon Center in community meetings and partnerships that enhance survivor support systems.
  • Stay informed on Transitional Housing Program policies, community resources, and available benefits to best assist clients.

Additional Responsibilities

  • Participate in an on-call rotation as needed.
  • Attend staff meetings, trainings, and community outreach events.
  • Perform other duties as assigned to support Avalon Centers mission.

Qualifications

  • Required: High school diploma or GED.
  • Preferred: Bachelors degree in a related field or significant professional experience in advocacy, social services, or housing support.
  • Strong communication, problem-solving, and organizational skills.
  • Compassionate, trauma-informed approach with a commitment to empowering survivors.
  • Ability to work effectively in crisis situations and with diverse populations.
  • Reliable vehicle, valid drivers license, and proof of insurance.
  • Must maintain strict confidentiality of all client and shelter information.

Physical & Mental Requirements

  • Frequent standing, stair climbing, driving, and lifting (up to 25 lbs).
  • Regular use of computer, phone, copier, and other office equipment.
  • Must be comfortable working in emotionally intense environments and managing challenging behaviors with professionalism and empathy.
