Position Summary

Collaborate with clients and families to identify available community resources to obtain critically needed services such as housing, benefits, and other entitlements.

Provide symptom monitoring, crisis intervention, advocacy, medication monitoring, transportation assistance, and ADL assistance.

Work with clients to develop and implement ISP goals.

Collaborate with professionals from multiple systems to address treatment needs.

Coordinate discharge with community agencies to transition clients from Central Connection to other service providers.

Employ goal oriented problem-solving techniques to assist clients in getting their needs met from a bio-psycho-social perspective.

Maintain appropriate documentation.

What We Value In Our Next Transitional Case Manager

Bachelor's degree in related mental health field or equivalent combination of experience and training.

Effective communication skills and the ability to interact professionally with persons from diverse backgrounds.

Possess knowledge of the Hamilton County public mental health system and other community resources.

Requires reliable transportation, a valid driver's license, and personal vehicle insurance for bodily injury/property damage liability.
