Schedule & Work Setting

After training: 5 days/week, 11:30 a.m.–9:00 p.m. ET

Business hours: 9:00 a.m.–9:00 p.m. ET

Hybrid: Work from home + occasional onsite meetings at Sandy Springs (Atlanta), GA

Home setup: Quiet workspace and secure high‑speed internet (cable/DSL)

What you’ll do

Assess member health status via inbound/outbound calls

Identify and address care gaps, coordinate services and resources

Educate on disease processes and self‑management

Encourage healthy lifestyle changes and plan adherence

Collaborate with Medical Directors on complex cases

Conduct post‑discharge follow‑ups to ensure timely care

Document assessments, education, and outcomes

What you’ll bring
Required

Active, unrestricted Georgia RN license

Willing to obtain multi‑state/compact licensure within 60 days of hire

3+ years RN experience (hospital/acute/direct care) or telephonic case management (payer)

Proficient with Word, Outlook, Excel, and web navigation; accurate data entry

Ability to attend onsite meetings in Sandy Springs (Atlanta), GA as needed

Preferred

BSN , CCM

Med/Surg, Home Health, Diabetes, Cardiac, or ER experience

Managed care and/or discharge planning experience

Chronic disease management across age groups

Salary: $58,800–$105,000 annually (full‑time)

Sign‑On Bonus: $5,000 for external candidates (eligibility and terms apply)

Pay factors include local labor markets, education, experience, and certifications
