SUMMARY
This position leads and/or provides oversight to a team of clinicians and support staff in providing direct clinical services to patients. This role may include assisting in orientation of new staff, role modeling, co-visiting to evaluate or demonstrate clinical skills, participating in quality assurance activities, identifying in service needs, designing in-service programs to address identified needs and supervising staff.
RESPONSIBILITIES
Ensures timely and appropriate assignment of service referrals and oversees the delivery and documentation of clinical services in alignment with agency standards.
Leads new staff orientation through structured training sessions, observational visits, and hands‑on case management support.
Serves as a clinical resource to staff, contributing expertise and supporting the development of specialized clinical programs and services.
Supports ongoing staff development by providing guidance in case management and clinical practices, and by monitoring and evaluating individual performance.
Participates in the design and delivery of in‑service training programs and communicates departmental updates through regular staff meetings.
Represents the agency at community meetings and upholds clinical and ethical standards, ensuring compliance with regulatory and legislative requirements.
Maintains member privacy in alignment with corporate policies and consistently demonstrates integrity by supporting the organization’s mission, values, and Code of Conduct.
Performs additional responsibilities as assigned by management and is expected to maintain regular and reliable attendance.
Leads the orientation of new staff by conducting orientation classes, providing observation and co-visits, and assisting with case management as appropriate.
Serves as resource person to agency staff in areas of clinical expertise and participates in development of special clinical programming as appropriate.
Helps to drive the ongoing development of selected clinical staff through provision of assistance in case management or other clinical services.
Oversees/guides staff in the provision and/or development of services to patients in addition to ensuring appropriate and accurate clinical documentation standards.
Monitors and evaluates individual staff performance.

REQUIRED QUALIFICATIONS
Master of Social Work
Licensed Master of Social Work or Licensed Clinical Social Work
PREFERRED QUALIFICATIONS
Community based knowledge
Home Care experience
One year of experience in a hospital setting preferred
PHYSICAL REQUIREMENTS
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
