With multiple locations across the U.S., Refuge for Women offers safe housing, at no charge to the resident, with around-the-clock care as clients progress through evidence-based, trauma-informed programming. Position Overview

Case Management consists of services that help clients gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case Management includes: (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated care plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation.


Your team contribution:

The Targeted Case Manager (TCM) carries out case management responsibilities in order to help the Emergency House residents take the next best step in their healing journey.

In this role, youll need both exceptional case management skills and the skills to connect individually with each of the women we serve. This role is more than just the tasks at hand and you sincerely care about walking alongside women and helping them make the next right decision to move toward healthy independence.



Responsibilities

  • Responsible for developing and implementing individual service plans, making community referrals, on-going reassessment of the service plans and follow up. Updates service plans as needed.
  • Coordinates all services for each individual client, including advocacy, giving information about community resources, etc.
  • Conduct group sessions weekly and assist with transportation to and from meetings
  • Provides accurate documentation of services, including identifying resources, writing progress notes, detailed summaries of all services and contracts and submits as required or outlined. Completes monthly progress reports and billing as required.
  • Meets weekly with Director for supervision.
  • Completes and submits case management notes weekly. Completes and submits other required paperwork per policy.
  • Completes documentation regarding all contacts and information regarding the case
  • Responsible for continued outreach and engagement of clients through telephone calls, home visits, and ongoing communication with all members of the program services team.
  • Oversees the collection and review of all collateral information including but not limited to: social histories, psychological evaluations, judicial records, previous substance use treatment information, DCBS reports, and obtaining a release of information for everyone involved in the case.
  • Responsible for assisting the clients in establishing transitional housing in the community
  • Ensures each client is assessed and completes enrollment for third party insurance and any other benefits for which they qualify


Skills and Expectations

  • Strong written and verbal communications skills including Word, Excel and Google email and applications
  • Strong project management, servant leadership and teamwork skills
  • Self-motivated, organized
  • Ability to adapt to frequently changing priorities and handle crisis situations


Qualifications

  • BA degree in human service field, preferably Social Work
  • Case Management certification in proper jurisdiction
  • 2 years experience in providing services to victims of crime including trafficking and sexual assault
  • Must have a valid state drivers license
  • Successful completion of a background check
