Job Summary
As Housing Support Case Manager, you will provide case management services to adults living in permanent subsidized housing to promote stability, wellness, and integration.

Duties and Responsibilities

Assist with housing search, leasing, move‑in processes, and furnishing purchases.

Conduct safety and wellness checks and housing safety inspections.

Develop a Housing Stability Support Plan and provide early intervention for rent disputes or landlord conflicts.

Educate tenants on rights and responsibilities and assist with annual housing recertification, inspection, or voucher conversions.

Carry a caseload of 25‑35 clients, making a minimum of one visit per client per week during the first three months and twice per month thereafter.

Provide care coordination, facilitate referrals to internal and community services, and train clients in independent living skills.

Recognize crisis situations and notify supervisor or clinical contact.

Document all service contacts within 24 hours to meet agency and state standards.

Participate in weekly team meetings, meet productivity standard of 40 billable hours per month, and attend performance reviews with program manager.

Perform additional duties as assigned by the team lead or director.

Working Conditions

Service delivered in community settings, Monday‑Friday 8:15 am‑5:00 pm; occasional evenings, weekends, and HSP crisis warm line rotations.

Required travel throughout DBHDD Region 3.

Must be proficient with computers, telephone, fax, and scanner.

Must pass pre‑employment drug screen and background check, hold a valid Georgia driver’s license, and complete agency-sponsored defensive driving course.

Minimum Qualifications

Bachelor’s degree in human services, psychology, counseling, criminal justice, sociology, social work or related field, or equivalent experience.

At least one year of experience in a behavioral health setting providing case management or community support service.

Proficient technology skills, including electronic medical record, telehealth, and mobile phone usage.

Strong care‑coordination skills and ability to link clients to agency or community resources.

Reliable transportation, strong interpersonal skills, and a positive team‑player attitude.

Pre‑employment drug screening may be required. Selected applicants will be subject to an FBI criminal history background check. Applicants may challenge the contents of their criminal history record if desired.
