Position: Full‑Time; Hourly/Non‑Exempt

Salary: $17.98 - $21.76 hourly (three levels based on experience and education)

Reports To: Miriam Gutierrez, Target Case Management Program Supervisor

Department: Case Management Program

Job Summary: The Success Coach (Case Manager) for Targeted Case Management (TCM) will provide specialized case‑management services tailored to individual client needs, create programs to benefit clients, and educate clients on available community resources. Success coaching ensures services are delivered in a timely, appropriate, efficient, and coordinated fashion.

What You’ll Do

Coordinate services between providers and participants.

Provide case‑management services and skill‑building support to youth in the program.

Provide housing case‑management services.

Maintain individual case records for participants receiving services.

Educate participants on available community resources and services.

Create programs and services to benefit clients and the community.

Attend community meetings relevant to program and clients.

Administer intake forms, assessments, intake packets, and service plans.

Conduct field visits, attend meetings, and be available to deliver services during traditional and non‑traditional hours in educational institutions, homes, courts, offices, outreach locations, and other approved settings.

Organize and facilitate life‑skills activities in both individual and group settings.

Participate in youth activities such as camping, sports, overnight conferences, and trips.

Requirements

Bachelor’s degree in Social Work, Family and Consumer Sciences, Criminal Justice, Psychology, Sociology, or Criminology from an accredited college or university.

Four years’ experience working with at‑risk youth or children in an educational, behavioral health, or similar setting.

Excellent written and verbal communication skills.

Strong ability to comprehend, interpret, and complete delegated tasks efficiently.

Must pass a CYFD Criminal Records Check.

Must maintain full certification in CPR/First Aid and CPI.
