Role
The Spine Clinical Care Coordinator facilitates an evidence-based, multidisciplinary, multi-specialty approach to care throughout the care continuum. Oversees the development and redesign, as needed, of patient education, of evidence-based standards of clinical care and the post discharge rehabilitation care. Gathers, records, and evaluates program data and uses this data in establishing program goals with the oversight of the Medical Director and the Program Manager. Accountable for the ongoing monitoring of program quality and assists with program marketing needs. The overall goal of the Spine Navigator is to improve organizational clinical/operational outcomes for spine-related conditions and procedures. The Advent Health Neurosciences Institute Spine Center model is structured to achieve established objectives using a team approach supported by a system-wide Spine Center Manager. The Spine Navigator guides interdisciplinary team members in performance improvement and the incorporation of evidence-based practice protocols into the daily operations of the Advent Health Spine Center Program. The Spine Navigator works with community and professional organizations to further community education about spinal care and treatment, and actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Responsibilities

Provides clinical mentoring for Spine Nursing Staff, implementing critical thinking strategies.

Reports clinical issues to the Manager and nursing administration.

Guides patients through spine surgery from pre-op education to post-rehabilitation, ensuring timely medical and supportive services via a multidisciplinary approach.

Manages appointment coordination, record requests, insurance concerns, education, and communication between the physician office and the patient.

Facilitates an evidence-based, multidisciplinary approach throughout the care continuum.

Makes discharge calls to ensure all care needs are met.

Leads monthly service line meetings, presenting clinical data and recommending improvements.

Stays current in evidence-based practice and emerging trends.

Develops Spine practice standards, including pre-op, post-op, and discharge instructions.

Creates patient education classes specific to spine surgeries.

Promotes the Clinical Coordination Program within the physician community and the public.

Participates in marketing through physician visits, dinners, seminars, and lectures.

Other duties as assigned.

Qualifications

Associate's of Nursing Required

Bachelor's of Nursing Preferred

Master's of Nursing Preferred

Three or more years of Work Experience Required

Team-centered leadership and facilitation skills

Ability to interact appropriately with department/teams to facilitate process development, quality improvement, and interaction with patients/families and the Clinical Coordinated Care Program Team

Strong organizational, communication, and customer relations skills

Ability to communicate effectively with diverse populations

Skilled in providing age-appropriate spine education

Flexibility in prioritization; ability to multitask; critical thinking/problem solving skills

Self-motivated and able to work independently

Public speaking

Proficient in Word, Excel, PowerPoint, or the equivalent

Registered Nurse (RN) Required

Basic Cardiac Life Support (BCLS) Required

RN Other Certification (RN) Preferred
