Estimated Salary: $60,249.48 $78,324.32 per year, depending on experience.
Description Intervenes with patients and families for psychosocial needs, providing assistance and support for coping with chronic, acute, or terminal illness. Services may include consultation, discharge planning, education, counseling, advocacy, and referral to meet emotional, social, and financial aspects of care. Provides crisis intervention for patients and families with psychosocial needs and coordinates a discharge plan of care. Acts as liaison to community resources, identifies psycho?social and environmental needs related to admission, diagnosis, treatment, and discharge, and collaborates with interdisciplinary team to develop comprehensive discharge plans. Makes appropriate referrals to community services, offers inpatient emotional support, counseling, crisis intervention, and bereavement services. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care to all patient groups: neonatal, infant, pediatric, adolescent, and adults.
Responsibilities
Intervenes with patients and families for psychosocial needs.
Provides consultation, discharge planning, education, counseling, advocacy, and referrals.
Offers crisis intervention for psychosocial needs and coordinates discharge planning.
Acts as liaison to community resources and identifies psycho?social and environmental needs.
Collaborates with interdisciplinary team to develop comprehensive discharge plans.
Provides inpatient emotional support, counseling, crisis intervention, and bereavement services.
Assesses and intervenes in child/elder abuse, domestic violence, and other abuse/neglect cases.
Provides care to neonatal, infant, pediatric, adolescent, and adult patient groups.

Qualifications Masters degree in Social Work (MSW).

Additional Qualifications Demonstrated ability to solve problems in a fast?paced environment.
01 year of social work experience in a hospital or related health?care setting; Emergency Department experience preferred.
Internship experience considered.
Excellent interpersonal communication and negotiation skills.
Strong analytical, data?management and computer skills.
Current working knowledge of discharge planning, crisis intervention, complex case management, and performance improvement preferred.
Strong working knowledge of community services available for insured and non?insured patients.
Strong organizational and time?management skills, with capacity to prioritize multiple tasks.
Ability to work independently and exercise sound judgment.
Ability to prioritize and manage multiple high?risk, complex patients.
Ability to work with multiple members of a care team and maintain positive working relationships.

Minimum required experience: Less than 1 year.
