Social Work Case Manager

Job Summary
The Social Work Case Manager collaborates with physicians, nursing, and healthcare team members to provide overall care coordination, focusing on psychosocial assessment and intervention, complex discharge planning, and leveraging community resources. The manager documents care plans, monitors psychosocial status, and ensures effective transition and discharge planning, facilitating patient and family coping and decision-making across the continuum to optimize quality of care, patient satisfaction, and cost.
Work Schedule
M-F; 8:00 am - 4:30 pm, with some weekend coverage typically every 6th weekend.
Qualifications
Education
Minimum required: Bachelor’s degree in Social Work or related field.
Preferred: Master’s degree in Social Work or related field.
Experience
Minimum required: None.
Preferred: Experience in Community Health or hospital continuing care/discharge planning. Three to five years’ acute care facility preferred.
Certifications/Licenses
Minimum required: Must have Wisconsin Social Work Training Certificate upon hire and complete required coursework within two-year timeframe, resulting in obtaining your WI State Social Work license.
Preferred: Wisconsin Social Work Certification and/or Wisconsin State Social Work license.
