Job Description Hope Haven is an accredited resource for disability services and advocacy. We offer service programs and support to meet a broad range of needs for children, adults, and families – from physical, mental, and intellectual to residential, vocational, and spiritual.
Hope Haven is growing, and we're looking for a Service Coordinator to work within our adult services. This full-time position will serve Spirit Lake and act as the internal case manager by developing person-centered plans, leading teams, and advocating for persons supported.

Responsibilities include:
Receiving referral packets
Arranging enrollment teams from prospective support teams, including lead staff from each service area (community living, employment, day habilitation, mental health, others)
Completing necessary assessments
Scheduling and leading initial and annual support team meetings, as well as additional meetings as needed
Developing person-centered supports and service plans focused on individual dreams and aspirations
Creating and maintaining comprehensive, up-to-date service plans and ensuring all supporting documentation (behavior support plans, crisis intervention plans, rights restrictions) are complete
Leading support team meetings and providing necessary training to staff
Completing required reviews, including quarterly progress reports, behavior support plan reviews, and rights restrictions
Enrolling and discharging clients in Credible, maintaining reports and demographic data
Managing the central file and conducting annual and monthly reviews
Serving as liaison with families, case managers, and other stakeholders
Securing and maintaining funding, completing all necessary paperwork for regulatory and funding compliance
Qualifications:
Bachelor’s or Master’s degree in Human Services or related field
Minimum 5 years’ experience working with individuals with developmental and/or psychiatric disabilities
Knowledge of impairments and their effects on personal and social functioning
Skills in supporting behavioral change
Excellent verbal and written communication skills
Basic computer skills
Commitment to integration philosophy
Willingness to complete agency training
Valid Class C driver’s license (or Class D if driving larger vehicles) and insurable driver status
Clear background checks and health assessments
Reliable transportation and proof of liability insurance
Physical Requirements:
Ability to walk, stoop, stand, sit, and climb steps
