Senior UAS Assessor Registered Nurse (RN)

Location: Bronx, NY (Field-Based, Hybrid)
Pay Range: $35.49 – $63.79 per hour + 8% Annual Bonus Potential
Schedule: Monday to Friday, 8:30am – 5:00pm EST

We are seeking experienced RNs to join our Long-Term Services & Supports (LTSS) team as fieldbased Assessors. In this role, you’ll conduct UAS assessments and develop care plans that directly improve quality of life for members with complex and high-acuity needs.

What You’ll Do

  • Conduct UAS assessments (2–4 daily) for members in the Bronx to evaluate needs and eligibility
  • Develop and update personalized care/service plans for members with medical or physical health needs
  • Educate members and caregivers on benefits, resources, and care options
  • Collaborate with providers, specialists, and care teams to ensure effective, person-centered care
  • Monitor and document member progress, conditions, and care goals in compliance with regulations
  • Serve as a trusted liaison and advocate between members, families, and providers
  • Partner with leadership to continuously improve quality of care and outcomes


What We’re Looking For

  • Active RN license in New York State (required)
  • Residence in the Bronx area (field-based position)
  • 4+ years of nursing experience (care management, MCO, or LTSS preferred)
  • UAS assessment experience strongly preferred
  • Strong communication, care coordination, and critical thinking skills
  • Ability to work independently in the field (personal transportation preferred; public transportation considered)
