Under Supervision of the Family Advocate Manager, the Safety Net Services (SNS) Case Manager plays a crucial role in child welfare, working to ensure the safety, well‑being, and permanency of children who are at risk of abuse or neglect. The SNS Case Manager will provide case management to families and assist with developing and implementing service plans with families aimed at addressing risk factors and promoting family stability.
Essential Duties And Responsibilities
Adheres to El Concilio California policies and procedures per the agency employee manual.
Conduct comprehensive intake assessments to determine client eligibility and needs.
Coordinate and refer clients to appropriate community resources including housing, food assistance, financial aid, health services, and employment support.
Assist program participants with the completion of any formal documentation that will provide access to resources such as housing, education, transportation, clothing, food, employment, etc.
Initiate referrals and advocate for program participants to receive adequate information to outside resources.
Build rapport using trauma‑informed and culturally responsive practices.
Manage a caseload of 30–60 program participants, balancing both short‑term and long‑term cases.
Conduct weekly contact with program participants via phone, email and in‑person meetings.
Maintain accurate and timely case records in the client database in compliance with agency policies and funding requirements.
Initiate and process purchase orders in accordance with the itemized approvals submitted by the assigned social worker.
Carry out home visits and deliver program materials while following El Concilio California’s established safety protocols for outreach and home visitation.
Collaborate with CPS social workers, internal team members, and external service providers to ensure continuity of care, including participating in Child and Family Team meetings when appropriate.
Prepare closure documentation that includes narrative summaries of engagement, progress, and referrals provided.
Participate in team meetings, trainings, multidisciplinary case reviews, and process improvement efforts.
Deliver educational presentations on diverse subject areas and resource options based on participant‑selected interests and needs.
Maintain professional and respectful relationships with all program participants, staff, and community partners.
Maintain current knowledge of de‑escalation techniques and best practices through ongoing training and professional development.
Required to work El Concilio California special events and meetings.
Other duties as assigned by immediate supervisor and administration.
Qualifications
Must be able to handle a high volume of documentation and task completion in a timely manner.
Maintain a cooperative working relationship with management, staff, and program participants.

Understand and work effectively with diverse cultural and ethnic groups.
Must complete and pass criminal and fingerprint background check.

Language: Bilingual in Spanish is highly preferred.
Education
A Bachelor’s in Social Work or related field with 1–3 years of experience working with families is highly preferred; An Associate’s Degree in Social Work or related field with 2–4 years of experience working with at‑risk population is highly preferred; A High School Diploma or GED Equivalency and 3–5 years of experience in case management is required in lieu of a degree.
