Job Overview This position coordinates utilization review service for defined patient populations across the acute care continuum. The role includes discharge planning, utilization management, care coordination collaboration, and support for resource utilization. It works collaboratively with an interdisciplinary team to improve patient care through effective utilization of facility resources.
Responsibilities Key responsibilities include:
Coordinate utilization review services for defined patient populations.
Manage discharge planning and utilization management processes.
Facilitate care coordination collaboration among interdisciplinary teams.
Support resource utilization to improve patient outcomes.
Qualifications Current licensure as a Registered Nurse (RN) in the state of California.
American Heart Association (AHA) Healthcare Provider CPR card (preferred).
Degree from an accredited baccalaureate nursing program (BSN) (preferred).
Certified Case Manager (CCM) national certification (preferred).
Interquel training required within six (6) months of hire.
Previous experience in at least two (2) areas of clinical specialty in an acute care setting.
Excellent communication, critical thinking, creative problem?solving, and organizational and planning skills.
Self?directed ability to tolerate frequent interruptions with a demanding workload.
Knowledge of hospital protocol and procedures, clinical standards and outcomes, funding options, community resources, federal and state regulations governing hospital and home care, and understanding of the financial structure of health plans and delivery systems (preferred).

Pay Range $49.47 - $71.74 per hour
