POSITION SUMMARY
Responsible for the effective management of Case Management services on Inpatient and Emergency Room. Oversees the RN Case Managers in terms of daily processes that assesses, plans, implements, coordinates, monitors, and evaluates to improve outcomes, experiences, and value. Assists Manager of Discharge Planning with Case managers scheduling and annual appraisals. Assists Manager of Discharge Planning with addressing developing staff and systems regarding workflows. Participates in planning and organizing policies and procedures in the Care Coordination Department. The Case Manager Supervisor will be assigned a primary unit responsibility 4 days a work week and will have one day (eight hours weekly) to dedicate supervisory administrative responsibilities.

EDUCATION
Bachelor of Science in Nursing. Current and Valid State of New Jersey License. Case Management Certification

EXPERIENCE
Minimum of five years\' experience in case management.

SPECIAL SKILLS
An experienced RN who connects patients with health care providers, coordinates appointments and treatment plans, and helps patients meet their optimum level of health. Current acute care clinical knowledge with broad bases medical/surgical background. Demonstrates leadership skill, effective interaction and communication (oral, writing, presenting) skills. Ability to function competently in stressful situation and a changing work environment related to changing patient needs. Ability to utilize effective problem solving and time management skills to set priorities perform job related responsibilities and respond quickly to unexpected situations. Ability to use critical thinking and clinical reasoning skills effectively.

Salary

Pay Range: $48.95 - $61.19 (per hour)
